I've never had one personally but am unsure how i would approach it and handle it particularly. What if she's really into the guy? Am i gonna feel jealous. What if he fucks her so good and she's loving it? Am i gonna feel jealous.
Biggest concern is just getting it started and feeling awkward and weird about it. Iam particularly interested in those that have had MMF as oppose to FFM

It went great. I'm not the jealous type so no problem. If you have insecurities about another guy being better or likely to have jealous thoughts then you need to think carefully before engaging in a ménage a trois.
