In the film Psycho Janet Leigh steps into the shower before turning it on! That water must have been stone cold, only a real psycho would do that.

I watched a medieval 50's movie and there was a battle scene where an army was clamouring up a hill where the opposing army were waiting at the top, they then started rolling huge tree logs down the hill at them. You can see stunt men actually being crushed and named, someone may have died in the process.
In the Elvis film Girls Girls Girls the scene where Elvis and a female co star transfer into a small fishing motor boat from a huge sailing yacht at speed, the guy steering the small fishing boat gets it wrong and steers it in front of the yacht by accident, it collides and Elvis/ Stunt double, I think it's actually Elvis gets clobbered by the bow of the yacht, the scene was kept in the film.
Another Elvis movie King Croele, Walter Mathau hits Elvis over the head with a chair, only it didn't break easy, and Elvis took it full force, he threw up after the scene because of the pain and shock.
A fight breaks out on a porn shoot where the actress loses her shit because another actress uses a dildo on her which she allergic too, anal/vagina burn. It was kept on the dvd outtakes.
Ben Hur film with Charlten Heston, the chariot scene looked too real and probably was, not surprised if someone died.
Vic Morrow, a co star of Elvis in King Creole, was in Twilight Zone movie and was actually decapitated.
Butch Cassidy and The Sundance Kid , the scene where they blow the safe on the train, they miscalculated the force of the explosion the actors/stunt men took it full force.
Ellen Burnstien had a rope harness attached and men yanked her across the room for effect in Excorcist when possessed Regan attacks her.
