A knife is first choice. If I am somewhere where a knife is not available, I would use the back of a spoon.

I've used a fork held back to front to do the butter, then switched it to mush the mackerel into the bread.

I have used a spoon when at a workplace, and there are no clean knives, no clean cloth nor washing up liquid available to wash cutlery in the sink, so used the back of a spoon on toast.
