The usual advice given I believe is to leave them in your gallery and ask Admin to review as mistakes can be made..

Ive never seen insertions on facebook lol... its the rules, if you believe youve seen worse then report the pics they mightve got through by mistake

It depends on what is inserted, normal toys are OK, anything that is outside of the norm isn't
