Is it possible for a user(s) to have a green flag but still be fake or not who they say they are account ?

In theory yes. If you think a profile isn't who they claim to be then report it.

Yes, always be wary.

I've got 20 in-person verifications.

But I could, for whatever demented reason, decide to tell you I was a tall slim blonde.

Nothing is foolproof and due diligence is always the right thing to do.

Loads of them. Normally guys with old relationship pics pretending to be couples. Quite sad really.
