Why when you go to a hospital follow up Appointment, Do you have to take a list of medications with you, when they are the people who issued the things.

They should have a record on the system but it's always good to keep your own records.

Last time I went to the hospital they asked me if I was on any medication , I said yes the ones that you gave me x
